Position fields where you need them

Full freedom to rename and rearrange fields. Names also display dynamically within the customization menu, as well as on the notification email you receive for a form submission.

Available Fields

Text

Single line text field used for basic information gathering.

Email

The email address field is required by Shopify for any contact form. We're unable to disable the option to delete this field, so be mindful that if this field is not present in your form, the submission will not work ⚠️

Number

Single line number specific field. Users will be instructed if anything other than a number is input.

Message

Multi line text field with click and drag resizing, providing a more user friendly experience over the one present in the standard form. You can also specify a character limit from 100 to 5000.

Dropdown

Dropdown menu with up to 6 options. Two are required, but the remaining 4 are optional. Simply leave the option title blank and they will no longer be visible in the list. There is also a custom heading and a 'please select' option that displays as default before a user has selected.

Selector

The same as the Dropdown menu, with 6 options only in a radio select format.

Checkbox

Checkbox allows you the opportunity to ask users to agree to terms or sign up for newsletters.

Select any of your color schemes

The form will automatically adjust to your selected color scheme, including contrasting submit button.

Set the options you need for Selector & Dropdown menus

Leave unused options blank to remove them from the form. You can also specify if users are required to select an option.

Works will ALL default Shopify themes running version 10.0 or higher
